Dementia Program

Our social worker will provide psychotherapy to address mood changes, as a result of memory loss, and family education on dementia and Alzheimer’s disease.

Pain Management Program

This program is a 13 week cognitive behavioral therapy approach for 1 hour weekly that will:

  • Reduce the pain associated with arthritis, back pain, headaches, and other conditions
  • Use scientifically proven techniques to manage your chronic pain
  • Reengage you in recreational, social, and work activities
  • Increase your productivity and improve your quality of life
Weight Management Program

This is a 13 week cognitive behavioral therapy approach for 1 hour weekly that will:

  • Normalize eating habits and conquer weight related fears
  • Change the way you think about your body and improve your self-image
  • Work towards positive change by taking control of your health

PROGRAM CERTIFIED STAFF

We have certified speech-language pathologists, occupational therapists, physical therapists, as well as a social worker that have an extensive background in this patient population. Our therapists have participated in specific team approach training designed to treat this patient population which helps us see the patient as a whole increasing our ability to help not only the patient but the family and the caregivers.

We also have a neuropsychologist on staff who attends our program weekly and performs neuropsych assessments in order to establish a cognitive and psychological baseline for the individual involved.  Our neuropsychologist writes behavioral plans, educates families on the individual’s specific deficits, and provides recommendations to the family for follow up.

Shad Duplessis is a Licensed Clinical Social Worker (LCSW) and Board Approved Clinical Supervisor (BACS) at Sage Outpatient Services providing individual, group, family, and couples therapy. He provides a comfortable therapeutic atmosphere for self-exploration and healing. Prior to Sage, Shad has 14 years of clinical and management experiences at all levels behavioral health care. He has worked in both long term and acute care inpatient and outpatient behavioral health settings in the Baton Rouge area. He has earned his bachelor’s degree from the University of Louisiana at Lafayette and master’s degree from LSU School of Social Work. At UL, he won the UL Scholar Award for his research on gender differences and the AmeriCorps Director’s Award. While at LSU, he studied abroad and was awarded the LSU Leadership Award and the National Association of Social Workers (NASW) Student of the Year for his work as the President of the Student Association. He also received specialized training on anti-racism at the regional and national levels and is committed to anti-racism principles today.


Dr. Rasmussen is the Director of Neuropsychology at Sage Rehabilitation Outpatient Services and is a consultant for Sage Rehabilitation Inpatient Hospital and Evergreen Life Services, and serves as an adjunct faculty member at LSU. Her experience and interests are twofold. She is licensed in neuropsychology and specializes in diagnosis and psychological treatment of neurological diseases and disorders including but not limited to dementia, mild cognitive impairment, traumatic brain injury, Parkinson’s disease, Multiple Sclerosis, epilepsy, and stroke. She underwent specialized training in memory disorders at Wesley Woods at Emory University and in neurorehabilitation at Emory Center for Rehabilitation Medicine and Lancaster General Hospital. She has published on the presentation of dementia in patients with intellectual disabilities and has done research on cognitive factors predicting conversion of mild cognitive impairment to dementia. In addition to neuropsychology, Dr. Rasmussen works in the field of health psychology, particularly with bariatric surgery patients and those struggling with chronic pain. She also obtained specialized training in these areas at Lancaster General Hospital. She helped develop a back on track program for individuals who have struggled with sustained success following bariatric surgery.
